Employee's arm caught in palletizer and injured

At approximately 11:30 a.m. on August 17, 2003, Employee #1 was cleaning loose water bottles from under the elevator of the Matthews palletizer. He reached through a floor opening, where two guard plates were missing, to retrieve a bottle when the case conveyor apron, on its return run, passed thru the area where his arm was, striking it hard and breaking both bones in this forearm. Employee #1 was standing by the palletizer's control panel wile reaching down through the opening and apparently did not push in the emergency stop button on the panel prior to reaching through the 7-in. by 2-in. opening, since the automatically activated case conveyor apron started up while his hand was in its path. The conveyor was tested on August 20, 2003 and it would not operate with the emergency stop button engaged. He was hospitalized and treated for serious fracture of both bones of the right forearm.
