Investigation Summary

Investigation Nr: 170838585
Event: 10/02/2003
Employee injured when falling off an ATV vehicle

At approximately 3:40 p.m. on October 2, 2003, Employee #1 and coworker were riding, back to back, side-facing on the rear package rack of a Honda "Foreman S" 4-wheel All-Terrain Vehicle (ATV), which was operated by a third employee. The vehicle came down a steep haul road and started a right turn, leading to the shop area at the end of the shift. As the operator started the turn, Employee #1, riding on the left side, apparently fell off the moving ATV and landed on his head on the haul road. The other rider first noticed Employee #1 had fallen off and alerted the operator, who stopped. Both employees ran to assist Employee #1, who initially was lying unconscious on his face and then they turned him over. Another employee called the emergency response team to transport him to Queen of the Valley Hospital. He was hospitalized for the head injuries he sustained.
