Employee injured in fall from roof

On November 26, 2002 Employee #1 was installing a roof on a small shed. The roof of the shed ranged from 8 feet to 9 feet high. A ladder was set up for the employee to access the roof, however, he decided that it was not stable and climbed up the side of the shed using a wood member and metal bracket, a method he had used previously. The employee placed his left foot on the wood member, held onto the metal bracket with his right hand, and reached for the roof. His hand slipped and he fell to the ground.
