Investigation Summary
On September 26, 2002, Employee #1, a laborer for an erosion control contractor, was part of a crew preparing to blow straw onto a bare cut bank at a subdivision construction site. Three coworkers were riding on a truck trailer that was hauling the Reinco power-mulcher and straw blower, Model Number M90, Serial Number 7951, to the bale pile. Employee #1 was trying to hoist himself aboard the slow-moving trailer when he slipped and his legs were run over by the vehicle. Employee #1 suffered a fractured left ankle and was hospitalized.
