Investigation Summary
On August 10, 2002, Employee #1, an end mill operator for Midwestern Pipeline Services, was in the end bevel repair area of the storage yard, clearing accumulated chips out of the cutting tools of a rented, field-portable CRC-Evans 36-inch hydraulic end-facing pipe-end milling machine. Employee #1 had his right hand on the rotation speed control and held a file with a hook on the end of it as the machine slowly turned, allowing him to pull chips out of the point of operation. A chip wrapped itself around Employee #1's glove, pulling his left hand into the point of operation and pulling his body to the left. This caused his right hand, which was on the speed control dial, to also move to the left, shifting the dial from LOW to HIGH. His left fourth finger was amputated at the first joint between the pipe and the cutter head. Employee #1 had failed to stop or lockout the machine before cleaning out the accumulated chips.
