Employee Injures Fingers on Table Saw

On May 8, 2002, Employee #1 was in a wood shop using a Powermatic table saw (Model Number 66, Serial Number 98367425) to rip small pieces from a 30-in. by 1.5-in. by 0.75-in. piece of wood. As he was working, his left hand contacted the 10-in. blade. Employee #1 sustained serious lacerations to his left thumb, middle, and fourth fingers, and nearly amputated his index finger. The saw blade was not guarded. At the time of the accident, Employee #1 had only recently been hired by the wooden cabinet manufacturer.
