Investigation Summary

Investigation Nr: 170381081
Event: 05/04/1994
Employee's fingertip crushed by closing gate

At approximately 11:00 a.m. on May 4, 1994, Employee #1, who had worked for one week as a mover/helper for Tri-Valley Transportation and Storage, Inc. was working with a bobtail truck. The truck had pulled away from the dock so that a lift gate could be closed. When the truck stopped, Employee #1 went to close the gate. He activated the lever to go up and engaged a pin to close the gate. Doing this put his finger in a hazardous area, and when he looked down he saw that his hand was bleeding and the fingertip of his left ring finger was crushed. He was taken to San Francisco General Hospital by his partner and released later that day.
