Investigation Summary
At approximately 6:30 a.m. on June 7, 1994, Employee #1, a foreman, entered a trench that was 8 feet 2 inches deep, 4 feet wide at the bottom, and 5 feet wide at the top. He was attempting to cut an old steam pipe with a gas torch. The soil composition was determined to be class C. There was no protective shoring system in place to prevent a cave-in. Employee #1 was only in the trench a few minutes when the side walls caved-in and buried him. Employee #1 was smothered and died of compressional asphyxia. The president said that Employee #1 was the competent person on the job site due to his past work experience. The company holds an annual safety meeting. The last one was held on Christmas, 1993. The company also relies on the steam fitter union 602 to provide safety training to the employees.
