Investigation Summary

An auger boom weighing approximately 5,300 lb was brought from one work site to another and reassembled. The crew connected the boom auger to a crawler backhoe and elevated a portion of the auger boom by placing it on top of two 6 in. by 6 in. pieces of wood that were 28 1/2 in. long. When Employee #1 arrived, the auger boom was already resting on the two pieces of wood, and he and the backhoe operator started to place the angling piston into the sleeve. The backhoe operator was inching the piston forward so the employee could put the end into a sleeve and place a pin through the hole to secure it in place. Suddenly, the auger boom lurched forward approximately 13 1/2 in. and fell to the ground. The top 6 in. by 6 in. block of wood had split in half and caused the boom auger to fall. The auger boom landed on Employee #1's feet. Although he was wearing steel-toed shoes, the employee sustained multiple broken bones to the top of his left foot. His right foot was not injured. 911 was called immediately and Employee #1 was taken to St. Agnes Hospital.
