Employee dies after fall from tree

Employee #1 was working in a tree approximately 35 ft above the ground and was re-crotching his climbing rope, which was not tied to his safety belt. He fell out of the tree, suffering several fractures and severe head trauma. He later died. The employee's lanyard was around the tree and attached to safety clips by a double hitch knot. The safety clips were connected to the D-rings on the safety belt. After the accident, the lanyard was found passing through one clip with a slip knot and the other side was straight with the clips still attached to the D-rings. The lanyard did not show signs of breaking; the rope was fairly new.
