Investigation Summary
On April 13, 1994, Employee #1 was working on top of a conveyor roller system with the corrugator department supervisor. The conveyor roller system is part of a corrugated super stacker machine that stacks sheets of corrugated paper that is used to manufacture corrugated boxes and sheets of paper. The machine stacks the corrugated paper onto wooden pallets, which are lowered to the conveyor roller system by the machine's elevator system. Employee #1 and his supervisor were freeing a pallet of paper that had become stuck on the conveyor roller system. Employee #1 stepped over a pusher arm on the conveyor and into an area that had several rollers missing. The pusher cycled and pinned the employee's lower left leg against the conveyor roller system table. Employee #1 sustained a double compound fracture to his lower left leg. He was transported to the shock trauma unit at the University of Maryland Hospital.
