Investigation Summary
Employee #1 walked behind a tractor that was backing up. The tractor, a Volvo FE7, was backing toward trailer #12261, which was 28 feet long. Employee #1 was crushed to death between the tractor and the trailer. The employee driving the tractor violated the company's rule of not stopping the tractor at a minimum of 10 feet from the trailer when backing up to it. He was fired for his actions. All drivers had been trained to stop their tractors at the appropriate distance. No ppe was in use.
