Employee's arm injured in fall from cornice ledge

Employee #1 was installing a high-tech material hoist on the flat roof of a brick building that was undergoing renovations. He was hoisting the first of two empty counterweight containers to the roof when the container became lodged against a decorative cornice. Employee #1 climbed over a 33 in. parapet wall and, while standing on the cornice ledge, began to push with his left hand on the wire rope to free the container. His right hand, which still help the pendant control, was hooked over the parapet wall. His thumb apparently pressed the START button and the hoist began to tip over the wall, striking Employee #1. He fell approximately 26 ft 6 in. onto the concrete walkway, shattering his left arm. Employee #1 was not using a personal fall arrest system or any other type of fall protection.
