Investigation Summary
Employee #1 had removed a decorative light from a smaller tree and was to re-install the light in a taller maple tree in the front yard of a private residence. The foreman pointed out the limb the light was to go on. The employee set up the aluminum extension ladder and climbed the ladder and then out into the tree on what he thought was the correct branch. The employee was wearing his saddle, tool belt, safety belt, and rope. When he got out into the limbs, he wasn't sure if he was on the right limb or not and started calling for his foreman, who was working in the rear of the house. The foreman could not hear him calling because of the lawnmowers being operated in a neighbor's yard. While out on the limb, Employee #1 sat down and checked for the best area to which he could tie off; he never did tie off while up in the tree. The branch he was on snapped and he fell 35 to 40 ft to the ground and the branch fell on top of him. He landed on his back and sustained a cracked T-11 disc in his back. Employee #1 was taken by ambulance to Suburban Hospital.
