Investigation Summary
On April 12, 1995, Employee #1, of C&S Hydraulic and Welding Services, Inc., was repairing the tracks on which a large recycling container moved. The container was located at Lorrax Recycling and Trading in Joppa, MD. He was assisted by a Lorrax employee. She was operating a Yale L83C forklift to push the container. Employee #1 directed her to give the container a "good push," but she did so not realizing that he had placed himself between the container and the exterior wall of the building. Employee #1 was caught between the container and the wall. He was transported to Fallston General Hospital, where he was pronounced dead on arrival. Both employees possessed appropriate levels of training.
