Employee killed in fall from scaffold platform

Employee #1 was on a partially planked scaffold platform, scraping mortar joints and windows. He fell approximately 40 ft to the ground and was killed. The scaffold was not protected by guardrails, and Employee #1 was not using any other form of fall protection.
