Investigation Summary
Employees #1 and #2 and seven other members of a carpentry/framing crew were working on a three-story condominium building. All of the trusses had been set in place and partially braced in various ways the day before the accident. Six flat trusses, which may or may not have been braced, were set over the rear breezeway and were to eventually hold twelve air conditioning units. A load of 44 sheets of roof sheathing, weighing approximately 2,053 lb, and 25 sheets of gypsum fireboard, weighing approximately 1,926 lb, were lifted by a Lull Highlander material lift and placed horizontally across the six flat trusses. Four employees were working up in the trusses getting ready to install the roof sheathing: two were away from the breezeway area and Employees #1 and #2 were at the breezeway area, where the six flat trusses had been set. For some reason, at least one of these trusses collapsed, knocking over the other five and two other trusses. A total of six trusses fell into the breezeway opening, and two were caught by ropes and wires and suspended in air. Employee #1 fell approximately 31 ft to the first-floor, landing on wooden debris; he suffered two broken legs and lacerations. Employee #2 fell approximately 10 ft to the third-floor concrete breezeway landing and sustained serious internal injuries. The collapse may have occurred because (1) the trusses were not braced or were braced improperly; (2) too much weight was placed on the trusses, causing them to buckle; (3) the wrong type of nails were used; or (4) a combination of all of the above reasons.
