Investigation Summary
At approximately 11:00 a.m. on August 10, 1994, Employee #1 was placing merchandise on the conveyor at the second-level of a distribution packing area. While lifting small boxes from a wooden pallet onto a storage rack, she fell 10 1/2 ft. Employee #1 sustained a laceration to her left ear, head abrasions, and bruises on her arms and legs. She was transported to V. M. Hospital, where she was treated and released that evening. Employee #1 had been given verbal work rules prior to starting the job. She could not remember exactly where she was standing when she fell.
