Investigation Summary
Masons were constructing a wall for a mezzanine area from a scaffold that was four frames high (27 feet) and 14 frames long (110 feet). The scaffold was properly erected with all braces and cross braces, and was anchored to the wall with the platforms fully decked and guardrails installed, except for on the inside of the scaffolding, which was against the wall being built. The inside edge of the fourth level platform was 15 feet from the mezzanine floor. The outside edge of the scaffold platform was 27 feet from the main floor and was guarded with standard guardrails. Two employees were on the platform stocking it with 8 inch by 16 inch concrete blocks. Employee #1 apparently suffered a seizure and passed out. He fell from the inside edge of the scaffold, struck the concrete wall, then fell to the mezzanine floor. Employee #1 suffered a severe head injury and was taken to the hospital where he was listed in critical but stable condition. The company does not have a safety or health program in writing. Its foreman conducts weekly training meetings with employees and keeps written records of the training sessions. Employees were trained a number of times on how to erect scaffolding and on safe work practices while working on the scaffolding. Employees were wearing safety glasses and hard hats. Employee #1's hard hat fell from his head during the fall.
