Employee injures arm and back in fall from 6 ft stepladder

Employee #1 had been assigned by his working foreman to help him install a fire alarm system in the building. The foreman instructed Employee #1 to work on the basement floor by himself and to replace the pre-existing wire by pulling the 106 ft wire for the smoke sectors and the door holders. Employee #1 was working on a new fiberglass stepladder when the ladder twisted and fell. He fell off the stepladder to the concrete basement floor, injuring his right arm and lower back. The working foreman called 911, who transported Employee #1 to the hospital.
