Investigation Summary

Investigation Nr: 200961225
Event: 06/13/1997
Employee injured in fall from scaffold ladder

Between approximately 8:30 to 9:00 a.m., Employee #1 and coworkers were adding a sixth level to an approximately 20 ft high fabricated frame scaffold. Employee #1 was on the far southern end of the sixth level of the scaffold, standing on a rung of the built-in ladder, putting the planks in place. Rain was falling lightly, and the scaffold was wet. Employee #1 was maneuvering a plank when he lost his balance and fell approximately 24 ft to the ground. He initially landed on his feet, and then fell onto his back. Employee #1 suffered bruises and a collapsed right lung. Emergency medical personnel were summoned and transported him to the hospital, where he was treated and released. Employee #1 was required to report back to the hospital the following day because of the collapsed lung.
