Employee injured in fall from elevated forklift

Employee #1 was retrieving a box of weatherstripping stored on a rack, 12 ft 8 in. above floor level. He was atop a pallet of doors, 80 in. by 34 in. by 30 in., elevated by a Yale forklift, model GLC050DENUAE083. The pallet was not secured to the forks and no guarding was provided for fall protection or for the exposed forklift mast. After Employee #1 moved the box onto the pallet, the forklift reversed, still elevated 12 ft 8 in. Employee #1, who was kneeling and facing forward, lost his balance and fell forward to the concrete floor, sustaining facial injuries and left hand trauma, causing temporary disability. He was hospitalized. Employee #1 was wearing safety glasses, but no means of fall protection were provided.
