Investigation Summary

Investigation Nr: 200962363
Event: 09/03/1998
Employee injures knees when conveyor overturns

Employee #1 and two coworkers were loading a trailer with plants. The OUT button was broken on the mobile conveyor, so one worker controlled the 'out' function from the other control box, located at the rear of the 40 ft long conveyor and on the opposite side. The conveyor was being moved out when its east side drive wheel struck the track, causing the conveyor to overturn. Employee #1 was struck as he tried to get out of the way. He suffered ligament damage in his knees, for which he was hospitalized.
