Investigation Summary
Employees #1 and #2 were helping to build a new single-family house. The foreman instructed them to install roofing tar paper on an 8-in-12 pitched roof. They were standing on top of the tar paper, which was nailed to the decked roof, when the paper started to move. Both employees fell 16 1/2 ft to the ground. Neither had been using fall protection, and neither had been trained in the hazards associated with working at elevations.
