Investigation Summary
Employee #1 and a coworker were on a roof canopy installing EIFS (dryvit) to a store sign. Employee #1, a plasterer with 25 years experience, was working on the left side of the sign and was kneeling on the top of a pilaster getting ready to match the other side of the sign's radius at the roof juncture. The unguarded and unprotected work space measured 15 in. wide by 39 in. long and had a piece of drywall board resting on top of it. The edge of the drywall, which was being used to close off drafts to a tarp-covered area, extended approximately 1/2 to 1 ft over the side of the masonry pilaster. According to a coworker, Employee #1 was adjusting his work position to receive a straight edge from the coworker on the other side of the sign. He moved too far toward the open side of the drywall edge, and the drywall that he was kneeling on started to break. In an attempt to catch himself he tried to grab the vertically-installed insulation panel. The styrofoam panel broke, and Employee #1 fell 19 ft 10 in. off the pilaster to the concrete sidewalk. He was killed. Employee #1 was not wearing any ppe and had not participated in any hazard recognition training or safety and health program.
