Investigation Summary
On January 19, 1995, Employee #1, a driller superintendent, and a coworker, a driller's helper, were at a job site where they had been working for approximately 24 months. They had been assigned by their project engineer to take soil core samples for a new housing project, and were locating premarked surveying stakes to drill bore holes for the samples. They had been at the job site since 7:30 a.m., and had located the last two bore marks for the day, #76 and #79. The coworker was operating an all-terrain vehicle that had a drill attachment on the back. Employee #1 was standing between two trees, approximately 45 feet away from the vehicle, directing the coworker as he put the ATV drilling machine in position. The coworker struck a 53 foot long dead tree with the ATV and pushed the tree over. The top of the dead tree broke off and struck Employee #1 in the head, causing severe head trauma and killing him instantly. Neither employee was wearing a hard hat or using the ppe provided. The weather all that day was cloudy and cold, with a misty rain.
