Investigation Summary
Employee #1--a foreman--and two maintenance men were moving and installing a Dusktop dust collector. Each maintenance man, operating a solid tire forklift, was making a tandem pick under a 2 inch angle iron flange of the collector's framework, which weighed approximately 1,000 lb, when the vertically positioned 42 in. by 50 in. by 3 in. high collector became unstable. The collector, which was 3 to 4 ft above the concrete floor, was being raised for a 40 inch high angle iron stand to be placed under it to support it at an elevated height. The unstable collector started to tip and Employee #1, who was directing the operation, tried to steady it, but the collector fell off the forks and fell on top of the employee. A projecting box used for the bag shaker mechanism prevented the collector from completely crushing the employee, who sustained three broken ribs on his left side and a punctured lung. The employee received no hazard recognition training and there was no competent person inspection. In addition, the load was unstable and no preventors were used. The employees were wearing hard hats.
