Employee struck and killed by 60,000 lb coil tractor

Employee #1 was performing maintenance work in a 56 in. tandem mill. While walking to the weld shop to get some assistance, he was struck and run over by a 60,000 lb coil tractor. He died from a crushing injury to the head and upper torso. The lack of training in and enforcement of a traffic control policy in the mill, and the lack of a safe walkway area for pedestrians, were found to be the major contributing factors to the accident. It was noted during inspection that the mill had only one safe walkway for pedestrians, which was located at the south end of the mill. Employee #1 was struck at the north end of the mill where the safe walkways are inaccessible.
