Investigation Summary
Employee #1 raised the boom from a vehicle with an electronic overhead crane. He then used a metal piece as a blocker to prevent the boom from falling and released the crane. When the employee was connecting the combustible tank below the boom, the blocker failed. The boom fell, crushing Employee #1's head. He died.
