Employee falls from elevated work surface and dies.

At 12:00 a.m. on November 22, 2016, an employee was installing decking. The employee fell from an elevated work surface of approximately 40 feet. The employee was working outside warning lines installed by the employer. The employee was wearing a harness, but was not tied off. The leg straps were not secured according to the Coroner.
