Employee killed in fall from roof

Employee #1 was on an approximately 3-in-12 pitched roof about 18 ft above the ground. He was placing roof trash into the chute of a dumpster when he fell and was killed. Subsequent investigation revealed that no fall protection had been provided around the chute.
