Investigation Summary
At approximately 1:30 a.m. on July 4, 2004, Employee #1 was told that he could go to sleep while a farm tractor was repaired. Employee #1 slept on the ground under a wagon. At approximately 2:30 a.m. the tractor repair was finished, and the tractor was hooked to a wagon. The tractor operator moved the tractor and wagon, felt a bump, and saw Employee #1 on the ground. Employee #1 was placed in a pick-up truck and taken to another location where a higher level supervisor was located. CPR was administered to Employee #1 at this location. Emergency services were telephoned at about 3:30 a.m. The coworkers thought Employee #1 had a broken leg and no life threatening injuries. The coworkers did not have a telephone available in the field to contact emergency medical services for assistance. However, they did have a two-way radio to contact their higher level supervisor. The employer had trained the workers on safe procedures to maintain farm equipment, including accounting for everyone prior to starting equipment. However, the onsite supervisor did not ensure everyone was accounted for prior to movement of the farm equipment. Employee #1 was killed.
