Investigation Summary
At 9:00 a.m. on August 22, 2016, Employee #1 was exiting a semi-trailer that he had been cleaning. The employee exited the truck by sitting down on the floor of the trailer, placing his foot on the bumper, and then jumping about 30 inches off the bumper onto the parking lot. Employee #1 landed awkwardly, resulting in a dislocation and fracture of his right ankle.
