Investigation Summary
At approximately 6:30 a.m. on July 29, 2016, Employee #1, an electrician, was assembling a main electrical panel while standing on the second step of a 4 foot stepladder. While pulling wires through a conduit above the panel he made contact with energized electrical components and fell from the step ladder. He was hospitalized and later died due to his electrocution.
