Investigation Summary
Employee #1 was in the back of a dump truck, unplugging a chute that was being used to dump scrap material from the roof to the truck. As he tried to pull the lower chute into a clear area in the truck bed, he bent the chute. Unknown to Employee #1, the chute contained 2,220 lb of material. When he bent the chute, the load inside shifted, causing the parapet wall to collapse. Employee #1 was killed when he was struck by falling bricks and debris. The chute was only rated to hold 2,000 lb of material.
