Investigation Summary
Employee #1 and a coworker were scheduled to place a pipe blind in a line, but the coworker apparently never received the message and Employee #1 was told to do the job himself. At some point during the job, Employee #1 apparently fell from the top of the tank on which he was working. His body was discovered at the base of the tank by another coworker. There were no witnesses to the accident.
