Employee dies when crushed by forklift truck

Employee #1, who was an order filler for the dispatch department of a roll paper warehouse, was assisting a coworker in filling an order. Employee #1 got on a Crown roll clamp lift truck model #40RCTT-S-190, serial # 1A112013, and headed to the unmarked aisle of mix roll paper area. He crashed into a metal fire wall, west of the mix roll paper area, and was found on the floor between his Crown roll clamp and the metal wall by another employee. He was crushed from his legs to his feet. Employee #1 died. Employee #1 had received fork truck operation training and was known to be a safe operator. No one observed the accident.
