Employee dies in fall through floor opening

At approximately 1:50 p.m. on January 16, 1997, Employee #1 and four coworkers were working 40 ft above the concrete floor in a paper machine building. They were using an air tugger and tag line to lower 200 lb, 4 ft by 25 ft aluminum panels. While guiding a panel forward, Employee #1 stepped into an approximately 3 ft wide by 20 ft long opening that was not fully covered by the panels that were already in place. He sustained multiple injuries and was killed. Employee #1 was wearing a body harness, but his lanyard was not tied off.
