Employee killed when fell from elevator

On September 24, 2001, Employee #1 of Perini Building Company, Inc. and a coworker were in a material and personnel elevator, which was running improperly due to the brake deficiencies. Employee #1 was trying to jump out of the car when he fell 24 stories to the base of the elevator tower and was killed. The coworker was trying to pull Employee #1 out when Employee #1's upper body was stuck between the car and upper frame of the elevator, but he was unsuccessful. The coworker was also injured but was not hospitalized.
