Investigation Summary
Employee #1 was standing with one foot on a 51 in. high dock and the other foot on a device used to lift the undercarriages of trucks. This device was in the up position (31 in. from the ground). A coworker inside the building accidentally pushed the button that released the pressure holding up the device, dropping it to the ground. Employee #1 lost his balance, fell, and struck his head on the concrete. He died of head injuries.
