Investigation Summary
At 11:52 a.m. on June 27, 2016, an employee was drying the rear of an idled washed car. The car switched into reverse, struck the employee, and then crush him between the car and another car that was emerging from the car wash tunnel on a conveyor track system. The employee had fatal blunt impact injuries to the chest.
