Investigation Summary
At approximately 4:15 p.m. on July 30, 1996, Employee #1 was repairing the stamper on a Mubea Ironworker machine. He lay down on the floor between the stamper and the machine carriage to tighten the bolts on the bottom of the stamper. Employee #1 was crushed and killed between the stamper and the machine carriage.
