Employee dies from head trauma after fall from ladder

At approximately 11:30 a.m. on October 5, 1994, Employee #1 was changing a marquee in front of a grocery store. He was using an 8 ft ladder to reach the bottom of the marquee, which is 10 ft from the ground with 3 ft of advertisement space above. Employee #1 fell 8 ft from the ladder, and struck his head on the asphalt drive. Employee #1 was rendered unconscious and was transported to the hospital, where he died from head trauma on October 10, 1994.
