Employee dies when struck by machine falling from forklift

Employee #1 was assisting a fellow employee in placing a machine on a fork truck. The machine was on the forks and was being raised to be moved out of the work area when Employee #1 yelled to the fork truck operator to lower the forks. As the operator lowered the forks, the machine started to teeter and then fell over, striking Employee #1 in the back. A number of fellow employees extracted Employee #1 from under the machine. Employee #1 was pronounced dead on arrival at the hospital due to internal injuries.
