Employee killed in trench cave-in

At approximately 8:40 a.m. on March 27, 1995, Employee #1, a laborer, was laying pipe in an approximately 11 ft deep trench. He left the trench shield to take a grade level check in an unshored and unsloped part of the trench. The east side of the trench collapsed, covering Employee #1 to his neck. He sustained a crushed chest and a rib punctured his lung. The operator of the tractor hoe went to his aid. When he ascertained that Employee #1 was dead, the operator then extricated him by using the bucket on the hoe. The coroner pronounced Employee #1 dead at the site.
