Investigation Summary
At approximately 10:30 a.m. on June 19, 1992, Employee #1 was using a crane to move a large bundle of 1 in. by 2 in. tubing that came from a mill. The tubing was to be stacked in the mechanical tube storage area in lane #4-E. As the employee transported the load, it swung and the chain around the bundle caught another bundle of 1 1/4 in. pipe, and pulled it down onto the employee. He was killed. The bundle of pipe weighed approximately 2,400 lb.
