Investigation Summary
At approximately 4:30 p.m. on October 5, 1994, Employees #1 and #2, of Trail West Trucking, Inc., LaGrande, OR, were on a highway, loading a tractor mower onto a trailer that was attached to a dump truck. The truck was parked approximately 2 ft outside the fog line. Employee #1 was standing between the fog line and the dump truck and trailer; Employee #2 was standing on the right side of the dump truck and trailer. As the employees secured the tractor mower, a semitruck with two trailers traveling westbound drove over the fog line, striking the trailer, Employee #1, and the dump truck. Employee #1 was killed. Employee #2 was struck by the loaded trailer and sustained internal injuries.
