Investigation Summary
At about 2:15 p.m. on January 29, 1993, Employee #1 was hit in the back and legs by a log measuring about 25 feet long. The log fell about 10 feet from a chain conveyor trough that runs from a Cambio barker to a chipper. Employee #1 was cleaning up an accumulated pile of chips and bark to the south and below the chipper. A coworker, the Cambio barker operator, had just run the log through the barker. The log then traveled by conveyor to the chipper. The log, being too large in size, stopped at the mouth of the chipper. Another coworker, the shovel operator, attempted to remove the log from the conveyor with the shovel. The grabbles on the shovel were too large for the conveyor trough and only allowed the grabbles to pinch the log. Attempting a second secure grab, the shovel operator rested the log on the side wall of the conveyor trough. The unbalanced log fell and hit Employee #1. He was transported by ambulance to Coquille Hospital.
