Investigation Summary
Between 5:50 p.m. and 6:30 p.m. on February 3, 1993, Employee #1 was fatally injured at a cedar shake mill in Coos Bay, Oregon. He was last seen working alone at 5:50 p.m. by a part- time carpenter. Employee #1 was a clean-up and utility maintenance man, working about 2 hours each evening. At the time of the accident he was cleaning sawdust and chips at the tail spool of an approximately 140-foot main waste conveyor. At about 7:15 the next morning, a third employee found him trapped in the tail spool. Employee #1's left hand had gotten caught between the bottom 19-inch wide conveyor belt and the tail spool. The belt and spool continued to turn, trapping his arm as far as the shoulder and forcing his face into the sawdust. His hips and legs were outside the upper and lower belt. He was pronounced dead and transported by the Millington Fire Department to the Coos County Medical Examiner.
