Investigation Summary

Investigation Nr: 712455
Event: 01/11/1993
Employee seriously injured when struck by logging mainline

At approximately 11:00 a.m., Employee #1, a hooktender, and a rigging crew were preparing to set the chokers on a turn of logs. The rigging returned to the brush. The rigging slinger blew a stop whistle, then blew the whistle twice, signaling the tightline to pick the chokers off the ground. The rigging was picked up about 15 to 25 feet above the ground. The rigging slinger then blew the stop whistle and the rigging stopped. Employee #1 grabbed the lead choker and proceeded under the mainline. Somehow slack got in the system and the mainline fell and struck the employee in the head. The rigging crew blew emergency whistles and first aid was immediately administered. The company radio was used to call a coast guard helicopter and the Myrtle Point ambulance; the emergency help arrived in about 15 minutes. Employee #1 was flown to Bay Area Hospital, then to Sacred Heart Hospital in Eugene, OR, where he was listed in critical condition.
